PAY AND BENEFITS

Annual Pay Range: $51,451.97 - $64,832.46
Hourly Pay Range: $26.385626 - $33.247415
• Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.
• Bilingual required positions receive an additional 5% compensation

JOB DETAILS AND QUALIFICATIONS
Clackamas County Health Centers - Behavioral Health Clinics is looking for Case Managers to work as part of the Community Support Team at the Stewart Community Center, located on the Red Soils Campus at Oregon City. We have one regular position and one bilingual required (Spanish/English) position to fill. The Case Managers will support and assist clients and families to obtain, maintain, and retain housing.
We are looking for energetic, resourceful, and empathic individuals who work well independently and as part of a team. The Case Manager in these positions must be very skillful in interacting and engaging with a diverse client base while maintaining professionalism in a stressful environment. The positions will include in-home visits and transporting individuals as part of a service delivery model.
The Case Managers will serve as a liaison between Health Centers and Housing Divisions while working with individuals and families to ensure long-term housing sustainability. Case management focus may include housing navigation, placement, maintenance and stability including paying rent, interacting positively with landlords and neighbors, managing behaviors that may accompany mental illness or substance use so they don't interfere with success in housing, developing crisis plans, connecting with appropriate community resources, and supporting clients' individual housing & health care goals. The Case Managers will provide skills training, case management, care coordination, & medication training.

Required Minimum Qualifications/ Transferrable Skills:*

• Qualified Mental Health Associate (QMHA) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO)
A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:
(a) Bachelor's degree in a behavioral sciences field; or
(b) A combination of at least three years relevant work, education, training or experience.
Additionally, QMHAs must demonstrate the ability to communicate effectively, understand mental health assessment, treatment and service terminology and apply each of these concepts, implement skills development strategies, and identify, implement and coordinate the services and supports identified in an ISSP.
• Bilingual skills (Spanish/English)
• Experience providing case management services or comparable function, working with homeless and/or low income populations
• Experience in assessing client needs and developing treatment plans
• In-depth knowledge, demonstrated by education, training and/or working experience, of homeless education programs, domestic violence, mental and emotional health issues including addiction, criminal justice involvement, short and long-term effects of poverty, oppression and homelessness on homeless adults and families with children
• Excellent interpersonal communication skills
• Demonstrated commitment and experience advancing equity and diversity in a workplace or community setting

• Experience working with adults with severe and persistent (SPMI) and/or substance use disorder diagnosis
• Experience with Electronic Health Records
• Experience with Medicaid billing

• Works with clients to develop individualized housing plans; assists clients with housing applications, completes supportive and subsidized housing paperwork, assists with navigating housing options and placement, including advocating for clients with prospective landlords. Monitors and evaluates each person's progression through their housing and Behavioral Health (BH) plan; develops corrective action revisions to the plan as needed.
• Provides case management, skills training, and care coordination services for individuals/families who receiving BH & housing supports; develops and coordinates resources to provide clients and potential clients with needed services; community outreach and coordination with service providers. Utilize agency approved Evidence Based Practices and outcome measures identified by CCBHC to monitor client progress in their recovery.
• Coordinates with health care providers, social service personnel, and other agencies to obtain housing supports and services; provides information and referral services to clients and families; advocates for individuals/families with other agencies for services. Identifies and collaborates across programs and organizations to serve the housing needs of the community.
• Determines housing, health, & social service needs of clients; interviews applicants, recipients, relatives, guardians, care providers, and other involved parties; conducts comprehensive needs assessments, including psychosocial, financial, and physical factors; evaluates functional impairment of clients. Make mandatory reports as defined by state law and agency policy.
This position works 40 hours during a standard workweek of Monday through Friday.
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
